Use this service to get a sick (fit) note for work following an absence of more than 7 days.
This will be ready for you within 10 days of the due date. It will be backdated to your requested start date if needed. If you have not heard from us within 10 working days from your due date, please contact us for further information. You may need to have a consultation with a clinician before this is issued – we will contact you for this.

- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
